MACHA SWAG DANCE (MSD) — TERMS & CONDITIONS & LIABILITY WAIVER

1. Tuition Fees & Additional Charges

  • The fees you pay at the time of enrollment are strictly for tuition/class fees only.

  • For any shows, performances, or events, students/parents may incur additional charges including but not limited to:

    • Costume rental fees

    • Performer ticket fees

    • Parent/Family audience tickets (optional if you wish to attend)

  • These charges vary based on the event and venue. MSD will inform all students/parents in advance whenever such fees apply.

CLASS POLICIES

2. Registration, Refunds & Attendance

  • No refunds, credits, or transfers once registration is completed.

  • Missed classes will not be made up unless the class was cancelled by the instructor.

  • MSD reserves the right to cancel or reschedule any class or batch as needed.

3. Enrollment Placement & Level Progression

  • All new students will begin at the Beginner level for their age group.

  • Instructors have full authority to promote or retain students at any level based on skill, readiness, and consistency.

  • Students may repeat a level for 2–3 sessions before moving to the next level.

4. Dress Code & Attire

  • Students must attend class wearing proper workout/dance attire (workout pants, t-shirt, etc.).

  • Dance shoes or sneakers are mandatory for studio floor safety.

5. Class Duration

  • Class duration varies from 30 to 90 minutes depending on batch and location.

6. Choreography Usage

  • Students require instructor permission to perform, recreate, or publicly share MSD choreography in whole or part.

  • This includes school events, weddings, cultural programs, or social media posting.

7. Studio Rules

  • Instructors, class times, levels, rules, and guidelines may change without prior notice.

  • All management decisions are final and binding.

8. Photo & Video Consent

  • By enrolling, you grant MSD permission to record, photograph, or video the student for:

    • Class activities

    • Promotions

    • Social media

    • Marketing or publicity

  • These materials may be used without compensation.

LIABILITY RELEASE & ASSUMPTION OF RISK

9. Acknowledgment of Risk

I/we understand that participation in dance and fitness activities carries potential risks including:

  • Strains, sprains, fractures, muscle injuries

  • Slip/fall incidents

  • Heat exhaustion

  • Serious injury or other physical harm

Despite MSD’s precautions, injuries can occur.

10. Release of Liability

By enrolling:

  • I/we voluntarily assume all risks associated with participation.

  • I/we release Macha Swag Dance (MSD), its owners, instructors, staff, and facilities from any liability for:

    • Injury

    • Property loss

    • Damages

    • Accidents occurring before, during, or after class

MSD is not responsible for lost, stolen, or forgotten items at the studio.

11. Medical Authorization

  • In case of emergency, I authorize MSD to seek medical attention.

  • I understand that I am responsible for all medical costs, regardless of whether I have insurance.

INFECTIOUS DISEASE WAIVER (INCLUDING COVID-19)

12. Health & Safety

  • Students (and accompanying parents) may not attend class if experiencing symptoms such as fever, cough, shortness of breath, muscle pain, sore throat, chills, or loss of taste/smell.

  • By attending classes, I acknowledge the risk of exposure to infectious diseases.

13. Waiver

I release MSD and its staff from liability related to exposure or illness arising from participation in classes or studio visits.

ELECTRONIC SIGNATURE AGREEMENT

By checking the “I Accept” box or signing digitally, I acknowledge that:

  • I have read and understood all Terms & Conditions.

  • I voluntarily agree to the Liability Waiver.

  • I accept all policies, risks, and responsibilities stated above.

  • This agreement remains valid for the entire duration of my (or my child’s) participation at MSD.
